Parent Declaration & Terms Agreement

By completing this booking, I confirm that:

☐ I have read and agree to the Another World VR Coding Club & Holiday Club Terms and Conditions.

I confirm that:

• My child is aged between 8 and 15 years and meets the minimum age requirements for the activities booked.

• All information provided, including emergency contacts, medical information, allergies, additional support requirements, and collection arrangements, is accurate and up to date.

• I have disclosed any medical conditions, disabilities, behavioural needs, sensory sensitivities, or additional support requirements that may affect my child's participation.

• My child is physically and emotionally able to participate in Virtual Reality, Augmented Reality, coding, gaming, and supervised activities.

• I understand that Virtual Reality experiences may cause dizziness, motion discomfort, eye strain, or disorientation in some individuals, and my child will notify staff immediately if they feel unwell.

• My child agrees to follow all staff instructions, safety briefings, venue rules, and equipment guidance at all times.

• I understand that unsafe behaviour, bullying, abusive language, deliberate damage, or repeated disruption may result in my child being removed from activities without refund.

• I authorise Another World VR staff to seek emergency medical treatment for my child if I cannot be contacted immediately.

• I understand that children must be collected promptly at the end of each session unless permission has been provided for them to leave independently.

• I understand that Another World VR cannot accept responsibility for loss, theft, or damage to personal belongings brought to sessions.

• I understand that photography and video permissions are managed separately through the registration process.

• I consent to the collection and processing of personal information for safeguarding, emergency contact, booking administration, and service delivery purposes in accordance with UK data protection legislation.

• I understand that cancellations, transfers, and refunds are subject to the club's cancellation policy.

Cancellation Policy

• More than 14 days' notice: Full credit transfer or refund.

• 7–13 days' notice: 50% credit towards a future session.

• Less than 7 days' notice: No refund or credit.

• Missed sessions due to illness, holidays, or non-attendance are non-refundable.

• If Another World VR cancels a session, a full refund, credit, or alternative session will be offered.
